Alexa, a Bay Area administrative assistant living with Postural Orthostatic Tachycardia Syndrome (POTS), never expected a video meant for her doctors and trainer to reach millions.
But after she posted footage of her Great Dane service dog, Otti, responding to a medical episode, the clip quickly spread across TikTok, drawing more than 7 million views and an outpouring of emotional reactions.

On the day of the now-viral video, Alexa had just left a cardiology appointment when her symptoms began to escalate. Her heart rate surged, and her oxygen levels dropped into the 80s. Before she could fully register what was happening, Otti already had.
As Alexa approached a long flight of stairs, Otti refused to let her continue. Each time she tried to move forward, the 120-pound dog blocked her path, recognizing the serious danger ahead. A fall in that moment could have resulted in severe injury. Instead of allowing her to proceed, Otti made a different decision, one that made his actions even more remarkable. Otti is far more than a companion. As Alexa’s second service dog, he plays a vital role in managing her daily life with POTS. He is trained to detect subtle physiological changes, including spikes in heart rate and drops in oxygen levels, often before Alexa is even aware something is wrong.
When he senses trouble, he alerts her, guides her to safety, and provides physical support if needed. His size, unusual even among service dogs, allows him to assist with balance and stability in ways smaller breeds cannot.
"I am so incredibly proud and thankful! He faced his worst fear to help me today. I don’t even know how he knew that was the only way I could safely get out of the building. He continually amazes me," she wrote on social media.
